Medicare Secondary Payer (MSP) is the term used when Medicare is not responsible for being the primary payer on a claim for medical services. Sometimes a commercial medical insurer, auto plan, accident plan, or work comp carrier may be responsible for first payment of medical costs. The burden for determining proper coordination of benefits (to prevent unexpected repayments and recoupments) begins with the provider.

With health insurance available from many sources, people often find that they have health care coverage under more than one insurance plan. They may be covered through their employer's health insurance program, have medical benefits as part of their automobile insurance or have coverage from another source of insurance in addition to coverage from Medicare. When a person has more than one source of health care insurance, the question often arises, "Who pays what?" The Federal Government has rules that decide when Medicare pays first, and when Medicare should only be billed after the primary insurance has paid.

Billing Medicare incorrectly can result in claims being retroactively denied and providers can be forced to repay considerable amounts of money, and even charges of fraud or abuse in some cases.
